National Caregivers Month/Resources

November is National Family Caregivers Month, a time to honor and support the caregivers who give so much to our families and communities. This month we raise awareness of family caregiver issues, celebrate the efforts of family caregivers, educate family caregivers about self-identification, increase support for family caregivers, and reduce feelings of isolation
 
Here are some local resources to help lighten the load, from mental health support to financial assistance programs.
 
Connecticut Home Care Program for Elders (CHCPE)
Care Management Services, Adult Day Health Services, Companion Services, Home Delivered Meals, Homemaker Services
 
Connecticut Statewide Respite Program
Offers relief to stressed caregivers by providing information, support, the development of an appropriate plan of care, and services for the individual with Alzheimer's Disease or related dementias.
 
The Caregiver Advise, Record, Enable (CARE) Act
Helps patients and their family caregivers when transitioning home from the hospital.
 
Paid Family Caregiver Program
Allows legal guardians and parents of school age children to provide and be compensated for certain waivered supports.
 
Money Follows the Person Program (MFP)
Provides transition services to help people move from institutional settings, like nursing homes back to home- and community-based settings.
 
Connecticut National Family Caregiver Support Program
Helps connect caregivers to services, offers short-term respite, training opportunities, purchasing items and services not covered by other sources, (as determined by the case manager) and connects caregivers to area support groups.
